Withdrawal from the University

To formally withdraw from the university, a student must:

1. Complete the online withdrawal form found on OASIS2.
2. Complete an exit interview with the Dean of Student Affairs office by calling 419-448-2062.
3. Obtain the withdrawal financial form from the Dean of Student Affairs, have the form signed by the Office of Financial Aid and the Business Office and return the form to the Dean of Student Affairs to complete the withdrawal process.
